Welcome to the Silver Lake Middle School Health Office!
All students are required by The Massachusetts Department of Public Health to have an up to date and complete physical examination and immunization record performed by a healthcare provider and conducted within the past 13 months on file at the school. An Emergency Contact/Annual Health form will be sent home to all students at the start of school and will required to be filled in and submitted back to school upon completion in the first few days of school.
Parents of incoming 7 students, please make sure you have submitted your child's most recent physical form with updated immunizations before the first day of school. This may also be faxed to 781-582-3564.
Grade 7 students will need:
Hepatitis B - 3 doses
Tdap Booster - Grade 7
DTaP/DT/Td/Tdap - 4 doses DTaP/DTP or >3 doses Td; plus 1
dose Tdap
Polio - 4 doses (4th dose must be given on or after the 4th birthday and ≥6 months after the previous dose, or a 5th dose is required. 3 doses are acceptable if the 3rd dose is given on or after the 4th birthday and ≥6 months)
Meningococcal - 1 dose, 7th graders
MMR - 2 doses
Varicella - 2 dose or physician documented history of chickenpox disease
If your student exhibits any of the following symptoms:
- Fever (100.0° Fahrenheit or higher), chills, or shaking chills
- Cough (not due to other known cause, such as chronic cough)
- Difficulty breathing or shortness of breath
- New loss of taste or smell
- Sore throat
- Headache when in combination with other symptoms
- Muscle aches or body aches
- Nausea, vomiting, or diarrhea
- Fatigue, when in combination with other symptoms
- Nasal congestion or runny nose (not due to other known causes, such as allergies) when in combination with other symptoms
If your child is going to be absent, please call the School Absent Line to report the absence. When a student is absent from school a note from a parent is required for the classroom teacher upon the student's return to school. A doctor's note is required in cases of absence from school due to an injury, communicable disease or an extended absence due to illness. Silver Lake Regional Schools asks that the health care provdiers specify any retrictions that need to be accommodated in school along with the date when full participation in the physical education program may resume.
Prescription and certain over the counter medications will require an authorization form to be completed and signed by a parent and physician before the nurse may administer. Your child's physician may use their own form or we have them in the health office. All medications must be provided in the original container with the current prescription label attached. The pharmacist will label a bottle for home and school if asked. Please note for field trips a separate labeled bottle will be a requirement. An adult must deliver all medications to school and sign in with an RN. Medications should never be sent to school with a child, including any over the counter medicine.
If your child has a health care pan for any health condidtions such as; allergies, asthma, diabetes, seizures etc. please submit applicable forms/orders from their physician or you can download the school forms from the website. If the physician allows them to self carry emergency medication this will need to be noted in the orders.
If you wish to opt out of in school screenings please put these requests in writing and forward them to the health office at the Middle School.

- SBIRT (7th Grade)
- Postural Screening (7th and 8th Grades)
